  • First and Last name (as it appears on passport)

  • Birth Date

  • Category - their current status(visitor/student/worker/refugee)

  • Phone number

  • Email

  • FULL Address (if they do NOT live with you)

      (street address, unit number if applicable, city, postal code)

We will also be requesting  documents for every individual such as their passport ID page (with issue/expiry date) and/or any letters from the government regarding their immigration applications/current status.
